HOME HEALTH

 

SERVICES PROVIDED
SKILLED NURSES - Registered Nurses and Licensed Practical Nurses, who work under the direction of your physician to provide you with the best care possible in your home.

HOME HEALTH AIDES - Well trained nursing assistants, who provide oral care, light housekeeping, ambulation, feeding, dressing, bathing, and other assistance as ordered by your physician under the supervision of skilled nurses.

PERSONAL CARE AIDES - Medically oriented personal care services provided by trained staff.

HOMEMAKER SERVICES - General household activities provided by a trained homemaker.

PHYSICAL THERAPY - Licensed physical therapist provide gait training, transfers, and strengthening exercises.

OCUPATIONAL THERAPY - Licensed occupational therapist provide training in activities of daily living and rehablitation services.

SPEECH THERAPY - Licensed speech therapist provide speech therapy to restore speech and the ability to swallow.

WHAT CAN HOME HEALTH DO TO HELP?

* Oncology Care
* Cardiac Care
* Pediatrics
* Pain Management
* IV Therapy, Tube Feedings
* Obtaining speciments for lab studies
* Injections
* Colostomy care and teaching
* Suture/staple removal
* NG tube insertion/changes/care
* Assess and teach medication regimen
* Foley catheter changes/care
* Assess nutrition and hydration
* Diet teaching
* Wound care/sterile dressing changes
* Assist in exercises (prescribed by a physician or physical therapist)
* Keep physician informed
* Teach patient and/or family to understand and participate in care
* Assist in or provide personal hygiene and homemaker skills
* Coordinate in other community agencies to provide a holistic approach to health care

 

WHO IS ELIGIBLE
1. Be under the care of a physician.

2. Require skilled services.

3. Be homebound (require assistance to leave your home.)

4. Require intermittent care.

5. Patient who has been discharged from the hospital with new instructions for care.

6. Patient who has wounds that are not healing or are infected.

7. Has a new illness or medications.

PAYMENT OF HOME HEALTH SERVICES

* Medicare and Medicaid payment is accepted as payment in full for Medicare and Medicaid receipents. Therefore, no cost is incurred to the Medicare or Medicaid patient for the services of Extended Family Home Health.

* Private Insurance Companies for those that provide home health coverage.

* Chanpus, Champ V.A.

All billing is processed for you.


Extended Family Home Health serves the residents of Pemiscot, Dunklin, New Madrid, Mississippi, Stodard, Scott, and Cape Girardeau counties.
